  If you have Microsoft Word Office 1997, 2000, 2003 or 2007 you can download the
  application and fill in the information needed, print a copy and mail it to the address
  shown below or you can e-mail the application as an attachment to:

  In order to e-mail the application once completely filled out, you must first save the
  document to your computer's hard drive, then prepare a new e-mail and attach the
  completed application file.

  Please note:  This application version is compatible with Microsoft Office 2007.  However, 
  the document must be saved in a Microsoft 2003 version or older.  When saving, please
